Title: Unusual Case of Recurrent Pleural Effusions: Thoracic Endometriosis Syndrome

Case Description: A 32-year-old female presented with recurrent right-sided pleural effusions. Initial imaging studies including chest X-ray and CT scan showed a large pleural effusion with no clear cause. A thoracentesis was performed and the fluid was consistent with an exudative effusion. The patient underwent video-assisted thoracoscopic surgery (VATS) to further investigate the cause of the effusion.

During the VATS procedure, a small nodule was noted on the diaphragm and was biopsied. Pathology results showed endometrial tissue. Further imaging studies including pelvic MRI and PET scan were performed and showed evidence of thoracic endometriosis syndrome, a rare condition where endometrial tissue grows outside of the uterus and can cause pleural effusions.

The patient was started on hormonal therapy to suppress the growth of the endometrial tissue and has had no further episodes of pleural effusion.

Key Takeaway: This case highlights the importance of considering rare conditions such as thoracic endometriosis syndrome in patients with recurrent pleural effusions. It also demonstrates the value of using a combination of imaging modalities and biopsy to accurately diagnose and treat these conditions.
